Central Registration (Admitting)

At the time of registration, you will be issued a Hospital Card. Please bring your Hospital Card and your Provincial Health Card with you on each and every subsequent visit to the Hospital.

Upon admission, you will receive a Hospital identification bracelet, which must be worn at all times during your Hospital stay.

The Hospital requires that each patient provide a credit card imprint (VISA or MasterCard) or deposit at the time of admission in order to guarantee payment of uninsured items (i.e. self-pay portion of a semi-private or private room, crutches, telephone, etc.).

All scheduled patients are pre-registered for their Hospital stay. Pre-admission forms will be provided to you by your doctor's office. Completed forms must be promptly returned to the Hospital Central Registration Department during the pre-registration hours of 7:00 a.m. to 8:00 p.m. daily.

In the case of a surgical admission, the surgeon will explain the surgical procedure to be performed and will ensure that the benefits and risks of the surgery are understood. A surgical information package will be provided with complete instructions to be followed. An "informed consent" form will be reviewed and you will be asked to sign this form and return all completed forms to the Hospital Central Registration Department.
